You have the right to make
decisions about the health care you get now and in the
future. An advance directive is a written
statement you prepare about how you want your medical
decisions to be made in the future, if you are no longer
able to make them for yourself. The State of
Illinois provides sample fill-in-the-blanks legal forms
for your use to declare your healthcare desires,
including the appointment of someone to act on your
behalf for medical decisions. This is especially
important in the case of domestic partnerships,
where the law and the courts do not yet recognize
partners' rights to make medical decisions for each
other. Please evaluate your options carefully and
consider preparing one or more of the following legal
forms:

Power of Attorney for Health Care
·
Declaration for Mental Health Treatment
·
Do Not Resuscitate Order
