FOR COMPLETING MEMBERSHIP/RENEWAL APPLICATION

Welcome to PRIME TIMERS TAMPA BAY, one of the most active and dynamic chapters of Prime Timers in the country. Your participation in, and enthusiasm for, Prime Timers will help assure our continued success as a club that meets the desires of its membership.

Important Notice

You must sign and date the Liability Waiver on page two.

When completing the Membership Application, please keep the following in mind:

1. If you have a partner living at the same address, each person needs to complete a separate application.

2. Any change in addresses (mail, e-mail), phone number, or other information should be sent to the chapter’s mail:

Please take time when circling the “Volunteer” (club services) and selecting the “Interest” boxes to make sure you have a reasonable commitment to each item.

Your responses will be placed in a computer database in order to produce lists of members with similar interests. Members will be notified of fellow Prime Timers with similar interests.
