This two page form contains the formal application and the Interest Survey that we use as a means to ascertain the needs of our members.

In either case download, print and fill out, make sure you sign the document and fill out the interest survey. Mail to the address supplied along with your check. Our 'family' memberships count two folks living at the same address.... you get one newsletter per month. Thanks
